Billing
When you have an appointment at our office you are authorizing the release of medical information necessary to process insurande claims to insurance companies for purposes of filing and payment. You are then authorizing the office to receive payment of medical benefits. You, as the patient, should understand that you are financially responsible for charges not covered, co-insurance charges, deductibles and non-covered services. Proof of insurance does not guarantee coverage.
